  9. REQUIREMENTS

    This permit is to be renewed annually.  Renewal reminder will be sent, however, it is the responsibility of the permit holder to ensure that the permit is renewed if a notice is not received.   This application must be completed and accompanied by a permit fee.

    I have read and completed this application and know the same to be true and correct and hereby agree to comply with all provisions of Article 5.03, Alcoholic Beverages, of the Northlake Code of Ordinances.
